R ESOLUTION N O. 2016-290
R ESOLUTION A PPROVING A PUD G ENERAL P LAN FOR “J ORDAN C ROSSING” LOCATED AT
15530 AND 15600 O LD R OCKFORD R OAD (2016037)
WHEREAS, St. Barnabas Lutheran Church has requested approval of a PUD general plan for
property presently legally described as Lots 1 and 2, Block 1, St. Barnabas Addition, Hennepin County,
Minnesota.
WHEREAS, the Planning Commission has reviewed said request at a duly called public hearing and
recommends approval.
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF THE CITY OF PLYMOUTH,
MINNESOTA, that it should and does approve the request by St. Barnabas Lutheran Church for a PUD
general plan, subject to the following conditions:
1. A PUD general plan is approved to allow a residential hospice facility to be constructed
southeast of the existing St. Barnabas Lutheran Church, in accordance with the plans and materials
received by the city on May 11, June 22 and July 28, 2016, except as may be amended by this
resolution.
2. A separate building permit is required prior to commencement of the project.
3. Prior to issuance of a building permit, the applicant shall submit the following or revise the
plans as indicated below, consistent with City Code, the Zoning Ordinance and the Engineering
Guidelines:
a. Execute a site improvement and performance agreement and submit related financial
guarantees and fees.
b. Submit executed cross access, parking and utility (water and sewer) easements between the
two properties and record them against the properties.
c. Submit an executed 10-foot trail easement adjacent to Old Rockford Road across Lot 2,
Block 1 St. Barnabas Addition.
d. Receive approval of final construction plans.
e. Execute an executed maintenance agreement for the filtration basin and pervious pavement
and record the maintenance agreement against the property.
f. Submit fire flow calculations.
g. Modify or vacate the access easement to the existing rain garden.
h. Revise the landscaping plan to increase the size of the overstory trees (maple, birch, and
hackberry) to the minimum planting size of 2.5 caliper inches.
i. Submit manufacturer’s specification for the pervious pavers that indicates the design
strength equal or better than seven-ton capacity for fire lanes and five-ton capacity for
parking spaces.
j. Identify the PIV location.

k. Provide a 1.5-inch maximum domestic line with an approved solenoid valve on a
combination six-inch fire/domestic water supply line to the building.
l. Revise the erosion control plan, as follows: 1) add a double row of silt fence along the
wetland; 2) flip the director of the filter logs; 3) Install heavy duty post silt fence around
aprons into filtration basin to avoid sediment from accumulating into the basin; 4) add a
note to keep vehicles out of the filtration basin; and 5) add a note to sweep at the end of
each day if tracking occurs.
4. Mechanical equipment must be screened in accordance with section 21130.05 of the zoning
ordinance.
5. Fire lane signage shall be installed in locations to be field identified by the fire inspector.
6. Standard Conditions:
a. No building permits shall be issued until proof of recording is confirmed for the required
maintenance agreements, trail and cross access, parking and utility easements.
b. Any signage shall require a separate sign permits.
c. The applicant shall remove all hazardous trees from the property at their expense.
d. No trees shall be planted in the public right-of-way.
