RESOLUTION NO. 2000-315
AWARDING THE CONTRACT FOR LONG-TERM DISABILITY AND GROUP
LIFE INSURANCE
WHEREAS, the City desires to purchase long-term disability and group life insurance
coverage; and
WHEREAS, the City has received proposals from National Life Insurance Company and
Fortis Benefits Company; and
WHEREAS, the City has determined that the proposal from National Life Insurance
Company is the lowest and best proposal;
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF
THE CITY OF PLYMOUTH, MINNESOTA, that National Life Insurance Company is
awarded the contract for the City of Plymouth Long -Term Disability and Group Life
Insurance.
Adopted by the City Council on June 27, 2000.
