RESOLUTION NO. 99-82
APPROVING AGREEMENT WITH APPLETREE INSTITUTE
FOR THE PURCHASE OF HEALTH/HOSPITALIZATION
INSURANCE PROVIDED BY THE SOUTHWEST/
WEST CENTRAL SERVICE COOPERATIVE GROUP
INSURANCE POOL AND BLUE CROSS BLUE SHIELD OF MINNESOTA
WHEREAS, the City desires to purchase insurance coverage for health/hospitalization
claims; and
WHEREAS, the City has reviewed the proposal for the necessary insurance coverage;
and
WHEREAS, the City of Plymouth has received proposals from HealthPartners, Medica,
and Appletree Institute; and.
WHEREAS, the proposal provided by Appletree Institute, the Southwest/West Central
Service Cooperative and Blue Cross Blue Shield of Minnesota is determined to be the
most comprehensive and competitive proposal.
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF
THE CITY OF PLYMOUTH, MINNESOTA, that the City Council approves the
contract for health/hospitalization insurance with Appletree Institute for insurance
provided by the Southwest/West Central Service Cooperative and Blue Cross Blue Shield
of Minnesota effective April 1, 1999 renewing January 1, 2000 and each January I't
thereafter.
Adopted by the City Council on February 16, 1999.
