RESOLUTION NO. 98-29
APPROVING THE EMPLOYEE FITNESS INCENTIVE PROGRAM FOR 1998
WHEREAS, the City of Plymouth Wellness Committee has proposed an incentive program to encourage
employee fitness and overall health;
NOW, THEREFORE, BE IT RESOLVED that the City Council of the City of Plymouth, Minnesota
hereby approves the Employee Fitness Incentive Program for 1998, based on the following guidelines:
Program Purpose. The City of Plymouth, through the Pathways Program, will reimburse employees for
health club monthly fees, subject to certain requirements.
General Program Guidelines.
• Eligible Enzployees All regular fiall time and part time employees of the City of Plymouth are eligible
to participate. Seasonal and temporaiv employees are not eligible. Members of the City Council shall
not be eligible.
• Membership at Health Club The employee must have an active membership at a health club. The
employee will pay the initiation fee. A portion of the initiation fee may be reimbursed to the
employee at the end of one year, if certain participation requirements have been met.
ReimbursemerztRequiremeli The employee must show proof from the health club that he/she has
had eight workouts on eight different days in a given month to qualify for reimbursement. The city
shall reimburse the employee$15.00 per month if the employee has met the participation guidelines.
Proof of monthly participation shall be submitted on a quarterly basis. The employee is responsible
for providing proof of participation. The city shall reimburse all participating employees at the same
rate of $15.00 per month, regardless of the category of membership.
Izitiatiorz Fee Reimbursement If, at the end of one year, the employee has had at least nine months of
qualifying participation, the city shall reimburse up to $100 of the initiation fee to the employee. The
employee must show a receipt for the initiation fee and nine monthly participation forms. Initiation
fee reimbursement shall not exceed the amount of the initiation fee.
BE IT FURTHER RESOLVED that the Employee Fitness Incentive Program shall be implemented on a
trail basis for 1998, that program costs shall be capped at $25,000, and that the program shall be
evaluated based on the following criteria:
--Number of employees enrolled in program, Januaiv, 1998
--Percentage of employees enrolled in Januaiv, 1998, who are still active participants,
September30, 1998. (Goal: 25% overall)
--Percentage of employees enrolled meeting monthly participation guidelines (Goal: M% each
n1onth)
BE IT FURTHER RESOLVED that the source of fitnding for the Employee Fitness Incentive Program
in 1998 shall be the Employee Benefit Fund, to be transferred from the Risk Management Fund.
