APPROVING CHANGE ORDER NO. 1
2001 MAJOR STREET MILL AND OVERLAY IMPROVEMENTS
CITY PROJECT NO. 1020
WHEREAS, Change Order No. 1 has been prepared providing for the following:
Modification to the pavement striping plan & crack repair. Addition of items
including: Bituminous Mixture for Patch Repair, Mill Bituminous Surface, F & I
G.V. Box Top Section, F & I Valve Box Insert Riser, Repair Irrigation System.
Contract Items were increased including: Constriction Drainage Strictures, and
Sodding. Revised completion date to September 29, 2001. Decrease items due to
modification.
WHEREAS, Change Order No. 1 for 2001 Major Street Mill and Overlay, City Project No.
1020, is recommended for approval;
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF THE
CITY OF PLYMOUTH, MINNESOTA: That Change Order No. 1 to Midwest Asphalt Corp.
for 2001 Major Street Mill and Overlay, City Project No. 1020, by adding $93,542.67 is
approved:
