RESOLUTION No. 2013-078
A RESOLUTION APPROVING PAYMENT NO. 5 AND FINAL
ANNAPOLIS LANE STREET RECONSTRUCTION PROJECT
CITY PROJECT NO. 11002
WHEREAS, the City of Plymouth as owner and GMH Asphalt as contractor, have
entered into an agreement for the Annapolis Lane Street Reconstruction Project, City Project No.
11002, said agreement was approved by the City Council on May 10, 2011; and
WHEREAS, the Engineer has certified his acceptance of the work and his approval of the
final request for payment.
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF
THE CITY OF PLYMOUTH, MINNESOTA:
1. That the original contract amount was $1,115,562.83 and the value of the work to date
is $1,022,901.01 and is herewith approved.
2. That the work performed by the contractor for the Annapolis Lane Street
Reconstruction Project and certified to the City Council by the Engineer as acceptable,
being the same, is hereby released from the contract except as to the conditions of the
performance bond, any and all legal rights of the owner, required guarantees and
correction of faulty work after final payment.
3. Request for Payment No. 5 and Final in the amount of $9,476.61 to GMH Asphalt for
the Annapolis Lane Street Reconstruction Project is hereby approved and the City's
Administrative Services Director is hereby authorized and directed to pay the same
forthwith. The project will be fiunded through the Water Fund ($94,545.86), Water
Resources Fund ($8,308.75), Sanitary Sewer Fund ($3,729.45), Municipal State Aid
Fund ($600,000), Street Reconstruction Fund ($149,019.94) and assessments
($419,591.59).
