REQUEST FOR PAYMENT NO. 1 AND FINAL
19% CRACK REPAIR PROGRAM
WHEREAS, the City of Plymouth as owner and Northwest Asphalt Maintenance, Inc.,
as contractor, have entered into an agreement for the 1996 Crack Repair Program, said
agreement being dated June 7, 1996; and
WHEREAS; the City Engineer has certified his acceptance of the work and his
approval of the final request for payment.
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF
THE CITY OF PLYMOUTH2% MINNESOTA:
1. That the original contract amount was estimated at $25,560 and the
value of the work to date is $29,374.83 and is herewith approved.
2. That the work performed by the contractor for the 19% Crack Repair
Program and certified to the Council by the City Engineer as
acceptable, being the same, is hereby released from the contract
except as to the conditions of the performance bond, any and all legal
rights of the owner, required guarantees and correction of faulty
work after final payment.
3. Request for Payment No. 1 and Final in the amount of $29,374.83 to
Northwest Asphalt Maintenance, Inc. for the 1996 Creak Repair
Program is hereby approved and the City Finance Director is hereby
authorized and directed to pay the same forthwith.
