APPROVING CHANGE ORDER NO.'S 3 AND 4
WEST MEDICINE LAKE PARK IMPROVEMENTS - PHASE I
CITY PROJECT NO. 330
WHEREAS, Change Order No.'s 3 and 4 have been prepared providing for the following:
Addition of trench stabilizing rock for the watermain, 4 six inch gate valves,
geogrid for roadway subgrade stabilization and disposal of excess subgrade
material. Also, the deletion of sidewalk along the west side W. Medicine
Lake Dr, and along the north side of 18th Ave. together with curb and
pavement quantities for the parking lot west of W. Medicine Lake Dr.
WHEREAS, Change Order No.'s 3 and 4 for West Medicine Lake Park Improvements
Project - Phase I, City Project No. 330, is recommended for approval;
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF THE
CITY OF PLYMOUTH, MINNESOTA: That Change Order No.'s 3 and 4 for West
Medicine Lake Park Improvements - Phase I, City Project No. 330, by adding $7,094 is
approved:
Original Contract Amount * $ 996,OU5.90
Previous Change Orders 44,855.00
THIS CHANGE ORDER NO'S. 3 & 4 - Add 7,094.00
REVISED CONTRACT AMOUNT $1,047,954.90
Adopted by the City Council on January 23, 1996
*NOTE; Change Order No. 1 with original contract - deduct $476,200.00.
