REQUEST FOR PAYMENT NO. i AND FWAL
IM CRACK REPAIR PROGRAM
WHEREAS, the city of Plymouth u owner and Northwest ARA& Maintenance, inc.,
as contractor, have entered law an agreement for the 1994 Crack Repair Program, aid
agreement being dated July 199 1994; and
WHEREAS; the Engineer has certified his acceptance of the work and his approval of
the final request for payment.
NOW, TIMUMORB, BE rr HEREBY RBSOLVBD BY THB CITY COUNCIL OF
THE CITY OF PLYMOUTH, MINNESOTA:
1. That the original contract amount was estimated at $50,000 and the
value of the work to date is $37,968.63 and is herewith approved.
2. That the work perforned by the contractor for the 1994 Crack Repair
Program end certified to the Council by the engineer as acceptable,
being the same, is hereby released ftom the engineer as acceptable
from the contract except as to the conditions of the performance
bond, any and all legal rights of the owner, required guarantees and
correction of huhy work aft final payment.
3. Request for Payment No. 1 and Final in the amount of $57,968.63 to
Northwest Asphalt Maintenance, Inc. for the 1994 Croak Repair
Program is hereby approved and the City Finance Director is beneby
authorized and directed to pay the sane forthwith.
Adopted by the City Council on September 12, 1994
