WHEREAS, Misdons Inc. Programs has requested a Conditional Use Permit
Amendment, Master Site Plan Amendment, Variance and Site Plan for Missions, Inc.
for a nursing home locked at 3401 Bast Medicine Lake Boulevard.
The property is more specifically described as:
That part of Government Lot 11 Section 23, Township 118, Range 22,
Hennepin Couoty, Minnesota, described as follows: Commencing at the
northwest corner of said Government Lot 2; thence on an assumed bearing of
asst along the north line of said Government Lot 2, a distance of 1030.00 fee;

degrees 32 minutes 37 seconds east 200.00 fee; thence south 31 degrees 17

WEMRBAS, the Planning Commission has reviewed said request at a duly called
Public Haring and nwommends spproval;
NOW,1HEREFORBw BE IT IPP RE11Y RESOLVED BY TPPB CITY COUNCIL OF
THE CrPY OF PLYMOUM, MINNESOTA, that it should and hereby don approve
the request of Misdons Inc. Programa for a Conditional Use Permit Amendment.
Master Site Plan Amendments VarWrce and Site Plan for a nursing home located at
3401 Boat Medicine Labe Boulevard, subject to the following conditions:
Compliance with the City Engineer's Memorandum.
2. Compliance of Policy Resolution 79-80 reganfing minimum floor elevations for
new structures on sites adjacent to, or containing any open storm water dninsge
fbcility.
3. Submission of required financial guarantee and Site Performance Agreement for
completion of site improvements for the nursing home within 12 months of the
date of this resolution.
4. Any signage shall be in complirnce with the Ordinance.
S. Any subsequent phases or expansfonr are subject to required reviews and
approvals per Ordinance provisions.
6. Compliance with the Ordinance regarding the location of fire hydrants and fire
Lutea and City specifications for drive aides.
7. Submission of required fire flow data and water main size supplying the site.
8. All waste and waste containers shall be stored in the designated trash and
recycling enclosure &Lown on the Site Plant and no outside wrap is permitted.
9. An 8 112 x l 1 inch "As Built" Fire Protection Plan shall be submitted prior to
the release or reduction of any site improvement bonds per City Policy.
10. The Master Site Plan of the campus for Missions Farms &MU be amended in
accordance with the plans received by the City of Plymouth on April 1, 1994.

11. The Master She Plan Amendment includes the location of two identification
dSm. The dxe of the ftas doll comply with the standards of the 2'bWq
Ordinance.
12. The request for a Variance for two id on d pa on the parcel meets the
Variance standards.
13. The she shall be maintained in a sanitary manner.
14. The request meets the standards for Cordhiooal Use Permits.
