REQUEST FOR PAYMENT NO.3 AND FINAL
NW -17 TRUNK SANITARY SEW13t, PHASE II
CITY PROJECT NO. 040
wHBRBAS, the City of Plymouth as owner and BTM Pipe SwAcet. 1ac.9 as
contractor, have entered into an agreement for NW -17 Trunk Sanitary Sewer, Pbase 11,
City Project No. 010, said agreement being dated December 7, 1992; and
wi1B1t W; the Bagineer has certified his accepts= of the work and his approval of
the final request for payment.

1. That the original contract amount was $193,600. Change orders were
approved in the amount of $2,520 and the value of the work to date is
$159,565 and is herewith approved. This project undeann the contract
amount by $36,555 or 18.6%.
2. That the work pedomed by the contractor for NW -17 Trunk Sanitary
Sewer, Phase n and certified to the Council by the engineer as
acceptable, being the same, is hereby released from the contract except
as to the condition of the performance bond, any and all legal rights of
the owner, required guarantees and correction of faulty work after fiat)
payment.
3. Request for Payment No. 3 and Final in the amount of $12,182 to BTM
Pipe Services, Inc. for NW -17 Trunk Sanitary Sewer, Phase 11 is hereby
approved and the City Finance Director is hereby authorized and
directed to pay the same forthwith.
