Plymouth Officially Reopens Zachary Community Playfield
With a snip of the scissors, Plymouth Mayor Kelli Slavik officially reopened the newly updated
Zachary Community Playfield, 4355 Zachary Lane, on May 21. City officials and representatives of
Plymouth -New Hope Little League gathered to officially celebrate the completion of a project that
converted two adult -size ball fields into three youth little league baseball fields. The project was a
collaborative effort of the City of Plymouth and Plymouth -New Hope Little League. Plymouth -New
Hope Little League contributed $250,000 to the project.

Plymouth to Extend Hours of Yard Waste Site May 24 — 28
to Accommodate Storm Clean Up
The May 23 storm ripped down trees and tree limbs through a swath of Plymouth
— and residents and city crews are dealing with the aftermath today. City crews will be
working steadily to clear away trees and branches that fell in the streets and rights-of-
way. Plymouth has extended the hours of the city -operated yard waste site through
Monday to accommodate residents needing to dispose of tree debris.
Residents are asked to make private arrangements for tree and branches that fell
on private property. City crews will clear trees and branches that fell in the streets and
rights of way. City crews will not pick up private property trees that residents have
cleared and placed in the right-of-way.
The City will offer extended hours at the Plymouth Yard Waste Site, 14900 23`d
Ave. N., from Thurs., May 24 through Mon., May 28 so residents may drop off tree and
brush waste. The Plymouth Yard Waste Site will be open 9 a.m. to 8 p.m. on those days.
Storm Damage & Clean Up
Storm damage was mainly isolated to two areas. The first area was east of
Vicksburg Lane, north of Co. Rd. 6, west of I-494 and south of Hwy. 55. The second area

was generally in the northeast part of the City, north of Rockford Road and east of Larch
Lane.
The City estimates that about 1,000 right-of-way and park trees were damaged.
The City expects to have major clean up done by the end of the day on Friday, May 25.
Xcel Energy reports that power is out to about 1,500 residents in Plymouth and
New Hope. Xcel expects to have power restored by 4 p.m. today.
Hiring Tree Contractors
Plymouth Forester Paul Buck advises residents to make sure that tree trimming
and removal contractors carry both liability and workers compensation insurance before
hiring them to work on their trees.

FIRE -RESCUE INCIDENT OF INTEREST
DATE: May 16, 2007 TIME: 3:46 p.m. ADDRESS: 10635 44a' Avenue
DETAILS: The Department was dispatched to a reported townhouse fire. Upon arrival, Duty
Crew Engine 21 (Plack, Hendrickson, Nordby, Keith) reported smoke evident within the dwelling.
A water supply was established and an 13/ " handline advanced into the dwelling from Side "A".
Upon further investigation, the fire was found in a lower level bedroom, and had been
extinguished by the fire sprinkler system.
Aerial 21 (Marti, C. Anderson, Ganje, Magy) were assigned ventilation and salvage upon arrival.
The balance of the assignment was cancelled.
The mattress and box -spring were removed from the dwelling. This fire was controlled by the
activation of one (1) fire sprinkler head.
RESPONSE TIME: Five (5) minutes. This call occurred during the Duty Crew's hours of
operation.
FIRE ORIGIN/CAUSE: The origin of the fire was in a lower level bedroom. Improperly
discarded smoking materials were the cause of the fire.
ESTIMATED FIRE LOSS: $10,000.

Dear Mayor Slavik and City Council Members Bildsoe, Black, Hewitt, Murdock, Stein and
Willis:
We are writing to express our heartfelt gratitude and appreciation for the outstanding
work done by emergency services staff at our home (4260 Quaker Lane, Plymouth). In the
early afternoon of March 30`h, our home became the center point of a stand-off between a
bank robber and law enforcement officials from several local cities. While this situation was
very stressful for us, we were comforted and impressed with the preparedness,
professionalism and genuine concern demonstrated by all of the responding emergency
services personnel
Far too often the work done by public employees goes unrecognized, is unfairly
scrutinized or even criticized. On March 301h, we observed service that should be appreciated
and applauded. We are truly grateful to each and every one of the individuals who played a
part in resolving this situation. There are a few individuals, who we had personal contact
with, who we'd like to acknowledge. These individuals exemplified the outstanding.service
and support we received from everyone. These individuals are Chief Goldstein, Detective
Voller, and Detective Kroeger. We'd like to take this opportunity to publicly thank them for
their outstanding service.
As those elected by the public to provide governance for the city of Plymouth, you
should be proud of the professionalism and level of service provided by the city's
emergency service personnel.

Thank you for your letter expressing your concern over the State of Minnesota application to the
US Department of Transportation's Urban Partnership Agreement (UPA) program. During the
development of the UPA application, the I-494 corridor was discussed as one of eight candidate
corridors for congestion pricing using dynamic shoulder lanes. For the final submittal, the
Minnesota UPA application highlighted the I -35W corridor with the 1-494 corridor listed as a
Tier 2 corridor meaning it will be considered for future study as part of a larger network. Before
proceeding with any of the tolling recommendations we would ask for community support along
the corridors.
The UPA program is a US DOT initiative to fight urban congestion. The US DOT is looking for
metropolitan areas interested in reducing congestion by utilizing aggressive strategies in the
areas of congestion pricing (tolling), transit, telecommuting, and technology. Through the UPA
program, up to $1.2 billion in grant funds is available for surface transportation over the three-
year FY07-09 period.
In response to the US DOT program, Mn/DOT and the Metropolitan Council have developed
Minnesota's UPA application submitted on Apri130"'. The focal point of Minnesota's
application includes the development of dynamically priced transit/auto lanes on the I -35W from
downtown Minneapolis to the Burnsville Parkway. The proposal includes converting the
existing High Occupancy Vehicle (HOV) lanes on I -35W to High Occupancy Toll (HOT) lanes,
future conversion of the HOV lanes planned as part of the I-35W/Crosstown reconstruction
project to HOT lanes, and the development of priced dynamic shoulder lane (PDSL) from 461h St
to downtown.
The UPA submittal also includes improvements for transit on Hwy 77 from I-494 to Hwy 62
including improvements on Hwy 62 connecting Hwy 77 to I -35W. Transit enhancements are
also proposed on I -35W north of Minneapolis to Hwy 10 and improved transit service on
Marquette and 2"d Ave. in downtown.
For the I-494 corridor, the option being proposed for further study through the UPA application
is the development of a PDSL. The concept of a PDSL is the reconstruction of a shoulder lane
that could be used during the peak hours or during times of non-recurring congestion. This

494 corridor was considered for the development of a PDSL because of the high levels of
congestion on the corridor and because planned improvements for the corridor were still years
away. The PDSL is meant to be an interim solution to provide immediate congestion relief until
funding is available for a permanent fix.
In the final UPA submittal to the US DOT, the Minnesota application included the I-494
corridors as a Tier 2 corridor. The Tier 2 corridors are included in the application to show a
region wide approach to congestion reduction initiatives. In addition to the I-494 corridor
through the City of Plymouth, the following Tier 2 corridors were also included in the UPA
Proposal:
• Hwy 77 —1-494 to Apple Valley
• Hwy 169 —1-394 to Minnesota River Bridge
• Hwy 36 — I -35W to I -35E
• I-94 — Minneapolis to St. Paul
• I-94 — St. Paul to I-494
Our proposal prioritizes Tier I projects for funding using the UPA grant money with intended
implementation by 2009. Tier 2 projects would be subject to further study and are not likely to
be implemented in the short term. Neither Tier 1 nor Tier 2 proposals would be pursued without
the support of the local communities along the corridor. I assure you that Mn/DOT will not
deploy a PDSL project on I-494 without working to gain the support of Plymouth and the other
communities along the corridor.

On May 20, 2007, I attended the annual meeting of the Mooney Lake Association.
At the meeting, I made a presentation about the upcoming construction project to
install a permanent emergency outlet system for the lake. The Association
generally is in favor of the project and had several questions and concerns that can
be addressed as the project design is completed: Some of the concerns raised
include establishment of a water elevation to activate the pump station, duration
of pumping during high water periods, noise levels during operation of the pumps,
replacement of the existing electrical panel with a cabinet, restoration after
construction is completed, and appearance of the pump station structure.
Minnehaha Creek Watershed District (MWCD) has retained the services of an
engineering consultant to complete the project design. City staff recently prepared
a newsletter (copy attached) for the neighborhood to provide an update on the
status of the project. A draft Cooperative Agreement has been prepared by
MCWD and is currently being reviewed by City staff. It is anticipated the
Agreement will be brought to the City Council for consideration in June.

The Minnehahp Creek Watershed District, with
input and assistance from the City of Plymouth
will begin taking steps to provide a permanent
outlet to Mooney Lake.
There is no natural outlet to the lake. This has
resulted in some damage to trees, yards and land-
scaping over the past several years. Homes have
not ---and are not currently—at risk for flooding.
However, heavy or persistent rain events have, at
times called for temporary pumping measures to
lower the level of the lake. These measures become
costly in materials, work and staff time according
to Plymouth Senior Engineering Technician.,

The proposed plan will create an outlet at the

Taylor cites several advantages to the location of
the proposed plan:
• The discharge pipe will connect directly to an
existing in-place storm sewer. This would
eliminate the possibility of an erosion problem at.
the outlet of the pipe from the lake.
• Most of the project would take place on City -
owned park property, along public streets and
within existing easements.
Project timeline
Workers from Wenck Associates, engineers for
the project, will begin survey work for the project
during the week of May 14, 2007. Survey infor-
mation will be used to draft plans for the project.

(DNR) will establish the level of the lake, which

SUBJECT: CERT (Citizen Emergency Response Team)
History:
Post 9/11, the President called for citizen volunteers to engage themselves in various
homeland/hometown security initiatives. This call created a surge and assisted in the further
development of an existing federal program known as the Citizen Corps Council. The Citizen
Corps Council's main goal is to encourage citizens to take personal responsibility for their
hometowns by preparing themselves for emergency situations and volunteering their time to
assist local public safety personnel.
The Citizen Corps Council is comprised of five different; yet related, volunteer entities: Citizen
Emergency Response Team (CERT); the Fire Corps; USA on Watch; the Medical Reserve
Corps; and Volunteers in Police Service (VIPS). Each entity provides an opportunity for citizen
volunteers to become involved in an area of public safety interest. For example, CERT teams are
designed to assist communities during the recovery from a natural disaster or significant
emergency event. The Fire Corps are citizen advocates for local fire department and USA on
Watch is tied into the Neighborhood Watch program. The Medical Reserve Corps was designed
for volunteer citizens with medical training and the VIPS program is equivalent to our Police
Reserve program.
Over the last couple of years, our Department was been asked about our interest in the Citizen
Corps Council. My contention was that the Department was already engaged with both the
Neighborhood Watch and Police Reserve programs. More specifically however; we were asked
by both Councilmembers and the State's Division of Homeland Security and Emergency
Management (HSEM) if we would be interested in launching a CERT program? At the time, I
indicated that the Department was involved with several other activities and that this program,
albeit worthwhile, would be a project for future consideration.

Last year, during one of the Emergency Management training sessions with Council, the CERT
program was briefly discussed. Councilmembers again indicated an interest in the program.
Therefore, based on the conversation, Captain Plekkenpol began to research what would be
required for the Department to start a CERT program.
In talking with the CERT coordinator at HSEM we were advised that there would be a grant
opportunity to fund the CERT program in the second or third quarter of 2007. With that
information, Captain Plekkenpol began his research and presented me with an overall guiding
plan in the first quarter of 2007.
The plan is quite simple: it is the Department's intent to apply for a CERT grant and if awarded
and accepted by the Council, we would like to train and equip 50 citizen volunteers who could
respond to our needs during. and after a natural disaster. Some of the training will include first-
aid, traffic direction, search and rescue, radio communications, etc. Their equipment will be
comprised of a backpack, helmet, flashlight, first aid gear, etc., and like the training, will be
covered by the grant. Upon passing the selection process and completing the required training,
the ongoing connection outside of an emergency or planned call -out e.g. Music in Plymouth, Fire
and Ice, etc., will be limited to three to four training sessions per year.
The thought is to break down the volunteers by the ward in which they reside and to identify
Ward Captains who will be in charge of coordinating communications and to begin the "calling
tree" if the CERT volunteers are needed. The four Ward Captains will report to one volunteer
Lead Coordinator who will work directly with staff to manage the CERT program.
Current Status:
Captain Plekkenpol has delegated much of the day-to-day CERT responsibilities to Sara Cwayna
who will be applying for the grant on behalf of the City. In order to apply for the grant, the City
must establish an oversight Citizen's Council. The Council will meet initially to establish certain
guidelines for CERT and will then monitor the program. This will not be a time -intensive
commitment. The invitations for the Citizen's Council were delivered and the response has been
very positive.
Next Steps:
Captain Plekkenpol and Sara Cwayna will be contacting those who have volunteered for the
Citizen Corps to establish their first meeting date. If the grant is awarded and accepted, the
selection process for CERT members will commence. Once selected, the training for CERT will
begin and it is our hope to have these volunteers ready for service beginning in late fall or early
winter.

FIRE -RESCUE INCIDENT OF INTEREST
DATE: May 16, 2007 TIME: 5:56 p.m. ADDRESS: 625 Xenium Lane
DETAILS: The Department responded to a reported entrapment at Olympic Steel. Additional
reports indicated that one worker was pinned beneath several steel plates. Upon arrival, Captain 1
(Evenson) confirmed entrapment and requested responding companies to stage lift bags, cribbing,
and other extrication equipment in preparation for rescue operations. Command was transferred to
Chief Kline, with Captain Evenson assuming the Rescue Group lead.
Duty Crew Engine 21 (Plack, Nordby, Hendrickson, Keith, Smith) were assigned as the Rescue
Group to begin stabilizing, securing the remaining load of steel, and to make the area safe for
rescue operations. The Worker was found to be pinned to the floor by approximately 4000 to 6000
pounds of flat steel product. The patient's left leg was beneath this load of steel. The work area
was very small, approximately 3.5' x 20' which complicated the rescue.
North EMS administered drug therapy through IV's and oxygen. A trauma surgeon was on -scene
if needed.
Several rescue plans were developed and equipment staged. The patient was removed by utilizing
the overhead gantry crane and two fork lifts to raise and support the load, while cribbing was
installed in a "crib as -you -go" fashion. The steel was -secured to prevent shifting of the load.
Following the removal of several coiled steel products and the positioning of needed equipment, a
slow and deliberate lift was accomplished to slide the patient out.
The extrication was completed 33 minutes following fire department arrival. The patient was
transported to North Memorial Hospital via Aircare helicopter.
RESPONSE TIME: Six (6) minutes. This call occurred during the Duty Crew's hours of
operation.
