Pursuant to due call and notice thereof, a regular meeting of
the City Council of the City of Plymouth, M nnesota was a on t e

Councilmember Schneider introduced the following Resolution and
moved its adoption:
RESOLUTION 4181- 226
EXTENSION TO THE PRESENT CONTRACT TO EMPLOYEE BENEFIT ADMINISTRATION COMPANY.
WHEREAS, the City has retained the professional services of the Employee Benefit
Administration Company to assist the City staff in the administration of its
self-insurance programs, and
WHEREAS, in the opinion of the City staff, excellent service at a reasonable cost
has been provided ov r the past several years, and
WHEREAS, the City has purchased a contract which provides group dental benefits
for all permanent employees, and
WHEREAS, the policy purchased is a self-insurance plan for all claims totaling
less than $750 per individual during the policy year.
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF THE CITY OF PLYMOUTH,
MINNESOTA that it should and thereby does extend the present contract for the
services of Employee Benefit Administration Company to assist the City staff in
the administration of its self-insurance Dental Program.
The motion for the adoption of the foregoing Resolution was duly seconded by
Councilmember Ho t *and upon vote being taken thereon, the
