RESOLUTION NO. 2007-428
APPROVING CHANGE ORDER NOS. 1-4
FERNBROOK LANE
STREET AND SIGNAL IMPROVEMENTS
S.P. 155-164-011 & S.P. 155-164-014
CITY PROJECT NO. 1013
WHEREAS, Change Order No. 1 has been prepared providing for the following:
Compensation to the contractor to fiirnish and install additional gate valves on
local distribution water mains; and
WHEREAS, Change Order No. 2 has been prepared providing for the following:
Compensation to the contractor to fiirnish and install two modular block retaining walls;
and
WHEREAS, Change Order No. 3 has been prepared providing for the following:
Compensation to the contractor to fiirnish and install additional gate valves on the
transmission water main; and
WHEREAS, Change Order No. 4 has been prepared providing for the following:
Compensation to the contractor to remove and replace gate valve boxes along with valve
bolt assemblies; and
WHEREAS, Change Order Nos. 1 — 4 for Fernbrook Lane — Street and Signal Improvements,
City Project No. 1013, are recommended for approval;
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF THE CITY
OF PLYMOUTH, MINNESOTA: That Change Order Nos. 1 — 4 for Eureka Constriction, Inc.,
for City Project No. 1013, by increasing the contract amount by $157,757.38 is approved:

FURTHER BE IT RESOLVED: Additional fiinding in the amount of $146,876.38 will be
needed from the Water Fund and additional funding in the amount of $10,881.00 will be needed
from the Municipal State Aid Fund.
