Pursuant. to due call and notice thereof, a, regular meeting of
the City Council of the City of Plymouth, M nnesota was Fe - o --n the

Councilman Seibold introduced the following Resolution and moved
its tion:
RESOLUTION NO. 771124
AWARDING CONTRACT, APPOINTING AUTHORIZED AGENT AND
TRANSFERRING FUNDS FOR THE PURCHASE OF LONG TERM DISABILITY INSURANCE
WHEREAS, the City has solicited bids for long term disability insurance coverage;
and
WHEREAS, Crown Life Insurance Company has submitted a bid in the amount of $.BO`
per $100 monthly payroll; and
WHEREAS, the protection afforded by such a policy is beneficial to City employees
and the City of Plymouth:
NOW, THEREFORE, BE IT HEREBY RESOLVED by the City Council of the City of
Plymcuth, Minnesota, that the contract for long term disability insurance
covering employees of the City of Plymouth for a one year period be and the
same is hereby awarded to Crown Life Insurance Company in the amount of
$:80 per $100 monthly payroll.
FURTHER, that the City Manager is hereby authorized to act as the official
agent of the City of Plymouth in the execution and admini!;tration of this
contract.
FURTHER, that funds be transferred from the General Fund surplus balance to
fund 101-192-821 Long Term Disability Insurance in amounts appropriate to
cover premium payments for the remainder of calendar year 177.
The msjtion for the adoption of the foregoing Resolution was duly seconded
